Some info about the day:

The #4 piston and rod caused some major damage. Four holes in the block/pan - one by the starter, one in the pan by the front, one huge hole in the back at the bottom of the block next to the pan, and one random hole above that.

The collateral damage is pretty serious, as well: The starter got taken out by a chunk of the block that got pushed out. The transfer case has a hole blown into it where the huge hole is in the back side of the block.

The biggest piece of piston left is the one pictured above (being held). It and the top of the rod look like they just exploded. The bottom 3/4 of the rod is still attached to the crank. The main girdle got massacred under cylinders 3 and 4. I'm sure that bottom part of the rod had a big part in the block destruction at 6300rpm.

The balance shaft assembly was intact. (@HPFPUPGRADE)

Spark plugs in cylinders 1, 2, and 3 were oil soaked. Oil was EVERYWHERE in the engine. The intake manifold had so much oil in it, that it was pooled in the bottom and coming out of the throttle body.

The spark plug in cylinder #4 was crushed, but the porcelain was held in, and 3 or 4 pieces were left to attempt diagnosis after wiping them off a bit. They need blasted with brake parts cleaner, but there was no apparent spackling on those pieces to indicate major detonation. I suspected this, as we didn't hear any pinging when the engine let go.

All of the boost control system lines were tight and in good condition.

I'm sure Daniel will have some more info, but I think we're in agreement that we don't have a good indicator of failure mode.

Wow. No evidence of recent detonation (at least yet) and no bent rods, explosive separation of piston from upper rod and destruction of piston. Maybe boost did get extreme in recent past before the tune being tested? Maybe way higher than reported 22 psi?

This is baffling. Wonder what the other surviving pistons and ring lands (especially top ones) will look like upon tear down.

Any damage up top in the valve train? Any evidence of jumping timing (valve crashing into the piston, etc.)?

I'm so sad for OP, but now it's time to get the car back in action and to see what can be learned from the failure. A teaching/learning moment.

We didn't pull the head or valve cover, but the timing chain looked taut based on a visual inspection from underneath.

too bad that when there is piston separation, there is no evidence left to diagnose it

hurts so bad to read the t/c took a hit. that's just insult to injury, and more paint to the wallet.

If you get a chance... check the crank end play and check to see if the BS assembly still rotates "clean". Its rare to see cyl 4 let go and normally that cyl will miss the BS assembly.

Have you checked the injectors? If you had an injector failure, that might be an indication to what the problem is.

We haven't done any more than drop the pan to look at the destruction. I suppose we could check end play if I dig out my caliper, lol. Not sure if the crank will rotate much after the destruction, but once the head is off we'll try.

I blame inadequate fuel pressure.

That makes a lot of sense and the ecu is going to do it's best to compensate for bad fuel pressure by increasing the injector's duty cycle, I know nothing of the disi injectors but regular injector don't like being pushed much beyond 80-85% duty, going beyond that they heat up and can possibly stay static open or closed or even altogether fail.

Sorry to about your misfortune op, sounds like dumb luck, Phate seems pretty damn methodical about the way he approaches tuning.

Did the OP still have the BS in the engine?

From what I've seen (unless I just happen to be focusing on these), most engines let up these days under WOT. I know that whenyou were here before, this was not the case, but it seems to be these days. We simply don't have nearly as many put put booms as we used to.

That sucks man. Sorry to hear. The boom looked/sounded pretty catastrophic, however, so I can't say I'm surprised.

On the plus side, you are now a full time Buick driver. At least it's not an Olds88.
